Information you provide

The youth waiting-list form asks for student and parent or guardian information, contact details, interests, availability, food allergies or dietary needs, and acknowledgments. Please have a parent or guardian complete the form for a student.

The volunteer form asks for contact details, interests, experience, availability, emergency contact information, and acknowledgments. Optional questions can be left blank.

How EFS uses your answers

EFS uses responses to contact families about the youth program, review interests and availability, plan student support and meals, and coordinate volunteer opportunities. Joining the waiting list does not guarantee enrollment.

How responses are handled

Submitted answers are stored through the website’s hosting and storage services. They are not published on the website. The submissions dashboard requires sign-in with Erica’s authorized account. Erica can review responses and download them for program administration.

Corrections and removal

To request a correction or removal, email ericafaye_00@yahoo.com or call 319.327.5044. Include the name on the submission and its reference number, if available. Please do not send additional sensitive details unless they are needed for your request.

Requests involving a student should come from their parent or guardian.

Videos and external links

This site includes video players and links to services such as YouTube, Facebook, streaming platforms, and donation providers. Those services may process information under their own privacy practices when you use their players or visit their websites.
